CHPA offers HIPAA-compliant Telehealth appointments
CHPA is taking precautions to lower the risk of COVID-19 transmission for our clients and staff during the evolving pandemic. If your provider has not made specific arrangements to see you in the office, your appointment will be by telehealth.
CHPA is currently not accepting new patients
For an initial appointment, please contact our front desk at 919.636.5695. Please review the required New Client Registration Forms and we will send you a link to complete and sign the forms online. If you prefer, print the forms and send them to us by U.S. mail or fax. You will be asked to review our Policies and Procedures and to provide your Insurance Identification card, if you wish for us to file your insurance claim.
