NOTICE OF DATA BREACH
Chapel Hill Psychiatric Associates, PA Notifies Patients of Data Security Incident at Third-Party Vendor Posted: 8.28.2026 This notice will remain posted through 12.6.2026 Chapel Hill Psychiatric Associates, PA ("CHPA") is notifying approximately 1,100 patients that their personal information may have been affected by a data security incident involving CareCloud, the vendor that hosts CHPA's electronic medical record system. What Happened CareCloud informed CHPA on July 2, 2026 that an unauthorized third party had accessed CareCloud's network and exfiltrated data between March 10 and March 16, 2026. CareCloud advised CHPA that it worked to determine which individuals and data elements were affected, and completed that assessment on June 24, 2026. Information Involved The information involved varied by individual. For the large majority of affected CHPA patients, only name and date of birth were involved. For a smaller number of individuals, the affected information may also have included email address, treatment location, date of service, provider name, medical record number, and/or CHPA patient number. For approximately 30 individuals, Social Security number was also involved. CHPA has been assured by CareCloud that no clinical information — including diagnoses, treatment notes, medications, allergies, or referring physician information — was involved in this incident. Credit card information is not stored within CareCloud and was not affected. What CareCloud Is Doing CareCloud is mailing individual notification letters to all affected patients describing the specific information involved in their case. CareCloud has established a dedicated call center staffed by trained representatives to answer patient questions about this incident. What You Should Do If you receive a notification letter from CareCloud, please review it carefully. We encourage all patients to remain alert to unusual account activity and unexpected communications requesting personal information, and to consider placing a fraud alert or credit freeze with the major credit bureaus, particularly if Social Security number involvement was confirmed in your individual letter:

CHPA offers HIPAA-compliant Telehealth appointments
CHPA is taking precautions to lower the risk of COVID-19 transmission for our clients and staff during the evolving pandemic. If your provider has not made specific arrangements to see you in the office, your appointment will be by telehealth.

For an initial appointment, please contact our front desk at 919.636.5695. Please review the required New Client Registration Forms and we will send you a link to complete and sign the forms online. If you prefer, print the forms and send them to us by U.S. mail or fax. You will be asked to review our Policies and Procedures and to provide your Insurance Identification card if you would like us to file your insurance claim.
